Pricing, Orders, Quotes, Repairs, Returns

Crestpoint is proud to serve as the one-source supplier to ophthalmic surgical teams for instruments, equipment, and supplies. We carry over 5,000 items from the highest quality manufacturers to meet every need at multiple price points. We offer both reusable and disposable choices. We have industry experts on staff as well as customer service personnel who specialize in the government and hospital markets. Our owners are world-recognized experts in the field of ophthalmic surgery and are on hand to help or discuss procedures or requirements and offer recommendations and educational services as desired.

Opening an Account

Once your account is established, credit approved and authorized purchase order received, we will confirm price and delivery schedule. Prices are subject to change without notice.

Request a Quote

To request a quote you can either call our Customer Service team, login with your account on this website, or click “Create an Account” at the top of the page, fill in a little information, and choose the products and quantities you are interested in. When finished, click the “Request a Quote” button, and an email will be sent to our Customer Service team who will respond to you promptly.

Payment Terms

Invoices are Net 30 Days unless mandated by law otherwise. Taxes applicable by location. We accept VISA, MasterCard, American Express, and check. Electronic funds transfer (EFT) accepted under certain circumstances only.

Shipping and Handling

Shipments by your courier of choice: UPS or FedEx. If no shipment preference is requested, UPS will be the courier used. Shipping and handling charges will be added to your invoice unless your shipping account number is provided.

Crestpoint Guarantee

All reusable products are guaranteed for life against defects in materials and workmanship. Any product serviced by other than the original manufacturer negates warranty.

Return Goods Authorization (RGA)

Repairs

Crestpoint offers a total Refurbish/Repair/Replace Program. Contact Crestpoint for an RGA (Return Goods Authorization) Number. Pack and protect the product in boxed packaging. Clean and sterilize before shipping. Ship product, prepaid shipping, insured via a traceable method, to Crestpoint Management, 9100 Watson Road, St. Louis MO 63126, with the RGA number clearly written in the attention line. After receipt of your repair/return product, and manufacturer completed inspection, we will advise price of Refurbish/Repair/Replace. Upon your approval to proceed, along with purchase order number, we will advise an estimated return ship date.

Inspections

  • Select instruments may be available to inspect, contact us for availability.
  • Instruments not previously purchased are available for a 30-day inspection period.
  • Exclusions include: cannulated instruments, custom made instruments, and diamond knives.

Orders for inspection are returnable if unused and in all original packaging. Sterilized instruments are NOT returnable. Contact us for Return Goods Authorization prior to returning.

Returns

The following items are not accepted for return:

  • Reusable instruments that are opened (not ordered for inspection only)
  • Custom instruments
  • Diamond Knives
  • Discontinued items
  • Sale or promotional items
  • Sterile and non-sterile disposable products
  • Specialty items
  • Cannulas and cannulated instruments
  • Sale items
  • Multiples of items are not returnable

Crestpoint accepts approved, unopened merchandise for credit toward product of your choice or credit toward your account. Item must be returned in its original unopened package, within 30 days after invoice date. Returns received after 30 days are subject to approval and a 20% restocking fee. Expedited freight charges are nonrefundable. When returning items for credit to your account, ensure to pack and protect the product in boxed packaging. Ship product, prepaid shipping, insured via a traceable method, to Crestpoint Management, 9100 Watson Road, St. Louis MO 63126, with the RGA number clearly written in the attention line.

Crestpoint offers assistance to the new surgeon, during the transition period from training to practice, with choosing product, tip styles, handle styles and special pricing on purchases. Please contact Crestpoint for more information.

Over time, changes in surgical techniques require increased use of intricate instruments/accessories that need routine inspection. Microsurgical instruments for ophthalmology have been miniaturized 10-50% in working end size and delicacy. Staff time to process, care for and procure instruments is reduced with cost efficiency. It is important to remove unused instruments from your trays for efficiency. A routine inspection program will help you save time and money. We offer economical Care and Handling Tips with a Refurbish/Repair/Replace Program to assist your staff.
